Bestway Medhub sets accounts up through its sales team rather than an online form. Send an enquiry or call telesales and they will get you started. They aim to respond within 48 hours.

Get in touch with Clarity to open a trade account. Their team will send you what you need and set it up. Accounts typically take a few weeks to open.
Contact Clarity →Phoenix / PSUK
For a dispensing practice, the Phoenix route is PSUK, its membership organisation for dispensing doctors. You join through a dedicated dispensing-doctor account manager rather than an online form.
